Facilitating the return home after hospitalization is the mission of Brittany Home Ker!
Located at the gateway to Lorient, our human-sized company provides proximity and personalized support for your patients.
Our binomial nurse coordinator / dietician moves throughout Brittany, for quality support.
And for any questions or concerns about the equipment, a call number is available for intervention 24 hours a day, 7 days a week (06-51-10-62-44).

Our actions in the care:
1. Pre-discharge visit of your patient in hospital service by our nurse or dietician.
2. Coordination of home care workers respecting the choice and habits of the patient.
3. Provision of the material necessary for the care, according to your recommendations. Delivery and installation at the patient by the nurse or the dietitian.
4. Training / information of the patient's IDEL in the home care and recall of aseptic rules.
5. Telephone calls and regular visits to patients' homes.
6. Regular telephone calls to IDELs.
7. Personalized accompaniment.
8. Written feedback ensuring fast and professional feedback.
